Enroll in Optional Critical Illness coverage up to $25,000 with no medical evidence required from August 1 - 31
The Benefits Trust is pleased to announce that our critical illness carrier, Chubb Insurance, has opened enrollment for guaranteed issue optional Critical Illness coverage. AOMBT plan members have mandatory basic Critical Illness at $10,000. During the month of August, Chubb will accept applications for additional Critical Illness coverage up to $25,000 without medical evidence**. Optional Critical Illness coverage can be purchased to supplement your basic coverage, or to provide coverage for your eligible spouse/partner. Coverage can also be purchased for dependent children. Click here for more information about Critical Illness coverage. We encourage you to consider your family’s needs and personal situation when contemplating optional Critical Illness coverage. As with other optional insurances, premiums for optional Critical Illness are your responsibility. Applications for guaranteed issue optional coverage will be accepted between until August 31st, 2017. Download the form for Employer Group Plan Members or Midwife Plan Members. The Effective Date of Coverage will be August 1, 2017.
**Usually, only new plan members are eligible for guaranteed issue coverage; all other optional coverage applications are subject to approval by the insurance carrier based on medical evidence.

As of August 1st, all Practice staff plan members and immediate family members can access the full suite of LifeWorks services. LifeWorks is an employee & family assistance program that includes including 24/7 telephone access, in-person or video counselling, financial and legal support, and hundreds of online resources, including toolkits, podcasts and calculators. All services continue to be free and are fully confidential.
