Prior to the benefits administration migration, all of our forms were the standard-issue versions provided by our third party benefits administrator. Because these forms were used by of their clients, most of whom are employers providing group benefits to their staff, there were some parts of the form that didn't quite fit for our midwife plan members. We've now completed the process of creating forms that meet the needs of our plan members, with versions for each plan member group. You can download the forms as PDFs and fill them out on your computer before signing them - you can even save the form if you can't fill everything out at once. To download the forms, right-click on the link and click "save as". You can access the general forms (Change forms, etc) on our home page, or grab the plan-member specific forms in the benefits page for your plan member group:

The AOM Benefits Trust Parental Leave Program supports the sustainability of the midwifery profession by reducing the financial hardship of those taking a leave from practice after the birth or adoption of their children.
Ensuring that the program is administered fairly and equitably requires regular review, analysis, and evaluation. This work is spearheaded by our Parental Leave Working Group (PLWG), comprised of Trustees, midwife plan members and AOMBT staff. The proposed revisions to the program were informed by an evaluation of the program conducted in 2014 as well as subsequent review of the Parental Leave program policy led by the PLWG. The upcoming changes are intended to recognize the complex ways in which midwives bill and work, increase the flexibility of the program, provide greater support for new registrants and those who have taken a parental or disability leave, and ensure that midwives are covered by the benefits program as soon as they return to active practice.
