2019 Member Information Report (MIR) now available online!
Your 2019 Member Information Report (MIR) is ready to view and download!
The Member Information Report (MIR) is an annual financial statement compiled by the AOM Benefits Trust providing midwife plan members timely, accurate information about their benefits funding and other programs which you participate in.
In previous years, the MIR has been mailed to you and available in print format. This year, the AOM Benefits Trust will be piloting a fully automated and online MIR. By automating the MIR, it allows you to access the information earlier than in previous years and represents a cost savings for the Trust.
The application to download the MIR is compatible with all desktop/laptop browsers and most mobile devices however, formatting may not be 100%. We will continue to roll out enhancements as we further utilize this application in coming years.
How to access your report:
Click the new Member Information Report button on the midwivesbenefits.ca home page to login and view your personal report.
- Your username is the same email address you use to login to the Portal
- Your default password is the first three digits of your Social Insurance Number and your year of birth (example: if your SIN is 789654378 and date of birth is 01/01/1970, your password will be 7891970). Once you have logged in, you will be required to reset your password.
The MIR is an important financial planning tool for midwives. It reflects your total benefits funding and highlights how these funds were allocated to the various components of the benefits plan.
You can easily print a copy of the MIR should you require a hardcopy, and, if applicable, you can also view and download tax receipts for your supplementary fees, and health and dental premiums. Your Group Retirement Savings Plan (GRSP) receipts for the 2019 tax year and first 60 days will be mailed to you directly from Desjardins. If you wish to access them sooner, you can download these by logging on to the Desjardins website at: dfs.ca/participant (click on "Your account").
T4As for Parental Leave payments and Leave Savings receipts (T3s) will be mailed out separately by the Trust. T4As will be mailed at the end of February, and T3s are mailed at the end of March.
