Operational updates Managing your practice

We know you are busy, and perhaps the last thing you (and the environment) need is a process that requires you to print and/or scan hardcopy forms. With this in mind, we are delighted to share that staff plan members can now complete, sign, and submit key AOMBT documents and applications online. All you need is online access from your computer or mobile device.

Practice Administrative Staff forms are available in the Employer Portal for each Midwifery Practice Group. Toronto Birth Centre and AOM Employee forms are also available on the relevant member pages on this website.
